Purchase of the 3 items must occur between 8/1/10 and 12/31/10. You need the UPCs off the 3 products, the form and your original receipt.
This would be a great way to turn your thrifty purchases into Christmas gifts!!!
I purchased some Band-Aids and Neosporin recently so I went back through my receipts to find them. Unfortunately, I purchased them on 7/22/10 - too early for the rebate! :( The form says allow 6-8 weeks for the book to be sent, and I want to make sure I have it in time for Christmas.
Instead of waiting for the next sale to come around on these items, since it states you can purchase ANY product, I'm going to hit the travel section for cheap travel size items!!
